Don't cover him up (I see that you say he is shivering, so I hope you aren't piling on layers and blankets, as that will only contain the heat). Keep with the Tylenol and Motrin, rub him down with cool washcloths, give him tepid-temp baths. If it continues on for a couple more days, take him to HIS doctor, not the ER. His doctor will have his records and be more familiar with his history, as well as having more resources to run diagnostic tests.

The motrin and tylenol should be give alternately every 4 hours. Tylenol then in four hours motrin. He's taking an antibiotic so the ear pain should be subsiding. One thing my ped's doc told me to try was powerade. I also do the popsicles, some ginger ale, Orange juice, apple juice. The fever is causing the chills, and he's been to the ER and has the medication he needs, now he needs time. Ear infections can be a real problem. I had them all my life and my youngest has them frequently, she had to get tubes put in her ears because of it. Lukewarm baths will help too. If these things don't work within 24 hours, call YOUR doctor and get him in. Keep him comfortable, cool but not cold. I'm mom to 3 and have been fighting high temps and ear infections for alot of years.

If the fever still isn't under control and hes isn't drinking, you mayneed to go back to the er. He may be getting dehydrated and need fluids to help him fight the fever. Experience tells me to try ice pops, or pedialyte pops, something to get the fluids into him. Ice chips were always a hit at my house. You can try putting a damp washcloth(warm, not cold) on the pressure points and back of the neck. If that doesn't help tonight get to the Dr. in the am.

Is he getting new teeth in? Maybe you should ask to be refered to a specialist other then taking him to the ER if you are not getting the answers you need. The temp is what I would be most worried about. Try pedialite if your son wont drink water. Also don't over dress him if his fever is that high. I would definitely get a second opinion. It could be that he has bronchitis or something else going on. Good luck !!!

the shivering is a good sign -it is the body's way of cooling itself. continue with the Tylenol every 4 hrs and Advil every 6 hrs. the antibiotics will help soon.
keep him cool -no bundling. make him drink. give him popsicles, let him play in the bath all day, make sure his cup stays full of water or pedialyte at all times.
relax and realize that he will get better soon. follow up with the pediatrician in a week.

Relax. I know that it's hard when your child is poorly, but remember that this time next week, he's going to be back to normal.
I'm not surprised that he won't eat or drink...he has awful earache...have you ever had an earache...did you feel like eating and drinking when you were in that pain?
Just empathise.
Offer him the drinks...don't bother with meals for a day or two, and keep dosing him up on pain killers.
You will get through this!

to get him hydrated try getting the Pedialite freezer-pops, he can chew or suck on them, the fever usually won't break if he's dehydrated. You might need to take him back to the ER and ask them to IV hydrate him if he won't drink.

at 104 - this is classed as dangerous / potentially life threatening
if your child is on meds - theres not alot that can be done - alternate between ibuprofen (child version) and calpol every 4 hours or so.
If he is this severe, I suggest you get him to the hospital and demand they need to help you out - tell them the temps your child is having - and I doubt they will turn you away
