Atlas Collars are meant primarily for neck immobilization. The "hole" you describe is simply an added feature... sort of a "secondary" function.

The "hole in the neck" area you mention is meant for access to a patient for emergency tracheotomy. I believe they used this collar and placed an emergency tracheotomy on Jason Priestly, of Beverly Hills 90210 fame, following his Indy Car wreck.
