Usually a physical examination of the genital area is done and sometimes a pap smear is done on women. The doctor will also take a sample of the fluid from the sores if there are any present.
You can also have a blood test done, but this is not always accurate and can sometimes provide false outcomes.
I was personally diagnosed with genital herpes over 6 years ago by my doctor. She gave me a physical and took swab samples. I never had a blood test done until recently. I had them test me for herpes about 4 months ago just out of curiosity, and it came back positive.

Laboratory tests can be used to detect the virus directly from skin lesions. A swab is rubbed over the sore and the material on the swab is sent for one of several tests called culture, FA, or PCR. Another test called serology can show evidence in the bloodstream for the patient's immune response to herpes infection. Both virus detection and serology can help confirm the health care provider's suspicions by physical exam and history that infection with genital herpes may have occurred. Genital herpes is common but it is difficult to diagnose just by physical exam and history. Most authorities feel an accurate laboratory test should be utilized to establish the diagnosis.

They can do a swab test of the sore if you are having what you think might be a break out, or they can do a blood test. The blood test can tell you what kind of herpes you have, but not where you have it. You can get both oral and genital herpes both on your mouth or on the genitals. Sometimes the tests (both of them) give false negatives.

I am absolutely terrified and I feel so alone. I just found out last week that I tested positive for hsv1 and 2. Sometimes, if the herpes are visible, the doctor can check it by just looking at it. The herpes sometimes aren't visible because it could be early warning symptoms or without lesion presenting or the blisters break and only open wound or scab left and can't check it by just looking at it. Then they need to confirm it by blood. With a strong light at the Gyn office, it's more visible than by just looking at it without lights. Visit your doctor and he/she will check it appropirately or otherwise, ask him/her to confirm it by blood tests. HSV1 can be in both areas as well as HSV2.
