Heart anomaly. Meaning a defect he was born with that no one knew about. Could have gone undiagnosed. Happens all the time unfortunately.

He may have had a heart defect that was undetected, and by exercise or stress, it triggered a heart attack.
Or, something as simple as having low blood sugar can precipitate a heart attack.
That is very sad...I heard recently of a 17 year old having that. He died. He was playing soccer and just crumpled over forward and died on the field. We think of older people with bad eating habits being most likely, so it's shocking when someone young and seemingly healthy has this happen.

I knew a 22 y/o California beach 'bum'. In the 60's, fit appearing, probably not eating a proper diet, tanned. Went to the ER because of a seizure. Found no BP in the right arm but normal in the left. He had further seizures and died of a stroke event. Autopsy showed a fully clogged R Brachial artery with atherosclerotic plaque. Cholesterol can do its dirty work at a young age!!

Could have been a case of arrythmogenic right ventricular cardiomyopathy, or A.R.V.C. This is an electrical defect that causes an arrythmia of the heart and in turn causes the right ventricle muscle to atrophy and become covered with fat...this is a leading cause of sudden death for young people...it also more prevalent in certain people of Italian descent.
