because the patient is incubated by needs of a trach tube via mouth nose neck so thats why there is a opening of the air way to breathe for them to make them breathe if they are unable to breathe

Um, not sure what you mean there, but if you are asking about the blue collar around the head, its there to secure the endotracheal tube in place. The endo tube does enter the trachea and goes into the lungs, then once secured is used to ventilate the patient or to protect the airway until the patient is able to maintain their own airway safely.
