I think that due to abdominal separation, your abdominal aorta is exposed to external pressures. It may burst as if an abdominal aortic aneurysm (AAA). You are in need of medical documents to prove that plastic reconstructive abdominal surgery is a necessity.
Only medical professionals can help you in the matter.
Please note that I am not a medical professional.

Your physicians should be able to provide medical documentation that you have this condition. They should also be willing to help. Or, a surgeon would be aware of the condition also. I would start there first. Tell them what your insurance company is telling you. Usually it only takes a doctor to write the insurance company to get this clarified. I don't understand why the insurance company would expect you to come up with proof that the condition exists. You should ask them, " Are you a medical doctor?" "So, are you saying that you are in the position to go against 3 medically trainded professionals on this issue?" "I'm sorry, what medical school did you say you went to?"
I smell a rat here. Sounds like the insurance company is fishing to find a way not to pay for this procedure. I believe that very well may be illegal. Especially if it is medically deemed necessary to correct this.
