Principle diagnosis is usually the 'main diagnosis' and in this case the heart attack is the main diagnosis.
If they weren't hypertensive before the MI, it could be a complication.
If they died, it could be a comorbidity.
HTN is not a procedure
I've seen MI and HTN both listed as admitting diagnosis

C. Comorbidity is a co-existing or additional medical condition. The heart attack would be the principle diagnosis.
