The best way to control the bleeding while doing the least damage to the fistula is to apply direct pressure to the exact place that is bleeding. The only time I will apply pressure outside of that area is sometimes there is so much blood that I've lost sight of the exact place to apply pressure, so I'll press down on the fistula on the arterial end (usually on the end closer to the hand). This will block the flow enough that I can remove the gauze and excess blood from the bleeding site and visualize where to put pressure. Once I've put pressure on the correct spot, I'll remove my finger from the arterial end. Putting too much pressure on too wide of an area of the fistula can be detrimental to the health of the fistula. That's why you should avoid carrying heavy objects, even purses, with your fistula arm, and that is also why you should only be applying pressure to the bleeding site.
If bleeding is not stopping even with applying direct pressure, call 911. If after 30 minutes of applying pressure the site keeps bleeding after you release pressure, reapply pressure and have someone drive you to the ER, or if you're alone, call 911.

RE:
Bleeding dialysis shunt (fistula)?
Just wondering, what is the proper way to control a bleeding shunt. Of course there is a high flow of arterial blood comming out of the site. I've seen a nurse apply arterial/venous pressure above and below the site of the bleeding shunt. Can I get confirmation on that?

What does the dialysis doctor say? Can they insert another shunt and close this one? Or repair the one he has?
I worked in dialysis for two years. There was a doctor who came through everyday and checked on each patient to see if everything was alright. What about the doctor who put the shunt in? Does he know about it?

My dad had a bleeding shunt and the nurse put pressure right on the site. The doc put some medicine right over it to try to get it to clot. When his blood test came back, they realized he was over anticoagulated, so they had to give him some medicine and put him in thehospital

Arterial blood loss requires emergency intervention from a qualified healthcare professional. I suggest you call 911 immediately if this is a real condition that you have.
