He needs to work with his doctor to find an approach that will work for him. Medication will help but often lifestyle changes can alleviate much of the problem. It might help for you to tell him how you feel, let him know that you love him and are concerned about his health and would like to work with him to lower his blood pressure if he will let you.

The bottom line is that there is no cure unless the High Blood Pressure is a side effect of something else in which case once the Doctors have sorted that out etc then Blood Pressure should reduce.
In this case if it is purely Hypertension (High Blood Pressure) then the only treatment is a mixture of medication and better diet, more exercise, healthier lifestyle.
The medication isn't problem, I have been taking medication for years now and quite frankly am feeling better than I have for along time.
High Blood Pressure isn't the death sentence it once was and the tablets are no big deal. A couple in the morning with a glass of water and by and large that's you fixed for the day. My BP was 261/160 when I was diagnosed and now it has dropped down to 115/75 (on a good day) but that is down to eating sensibly, cutting back on the alcohol (I still have been known to "go for it" from time to time) and luckily I was never a smoker so that wasn't a problem.
Try and accomplish things in stages. To be honest your Grandfather is probably pretty set in his ways so sending him down to the gym on a daily basis is a non starter but get him out for walks every day, eat slightly smaller portions of food, start eating a slightly changed diet - he will notice the difference and feel the benefit and once that happens then he will "buy into it".

I'm sorry that your grandfather is being uncooperative, unfortunately there is no cure. However you can try to change his diet and limit his salt intake. Try fresh fruits and vegetables no canned or frozen foods and no alcohol. If he does not want to take his medications because of certain side effects, please go back to the doctor, there are many many different kinds of blood pressure medications, perhaps a simple switch can make him happier and compliant with a different medication. Lower sodium,cholestrol and saturated fat intake levels. There's no cure for it. Once you have it,you take your meds and eat/live better,or you...well. you won't be living long at all.
Hopefully he'll make a good choice. Tell him to drink at least 10,8 oz glasses of water a day,too. That'll help.

If he is having side effects to his medication he should discuss this with his doctor there are many others he could try.
Unfortunately we cannot stop other people from behaving stupidly.
In the final analysis it is up to him to take care of himself
