A mild heart attack may just involve a smaller artery and area supplying the heart. It may not even be noticed in the form of chest pain, pressure, or shortness of breath. A massive heart attack can be fatal. It usually occurs in one of the three major arteries supplying blood to the heart and will cause severe heart damage.
