It could indicate a number of things. It could be indicitive of previous illness such as pluerisy or pnuemonia. From a collapsed lung, smoking too much, being exposed to a toxic chemical such as tar (smoking again!), asbestos, from working in a mine, soot etc. It does mean that something had to damage the lining in your lungs somehow, so it's usually noticeable after the incident which caused it.

Well, a doctor would wind up asking you TONS of questions about what you have exposed to your lungs. Like someone else answering you said, the asbestos, the coal, even just grinding stone you can get silicosis (condition where there is too much silicon-main ingredient of quartz and many other rocks-in your lungs. If you really have never exposed your lungs to anything you are aware of (clove cigarettes are another really really bad one...it can actually blow tiny holes in your lungs) then I would suggest you check your family for a history of Cystic Fibrosis and get tested for it even if no history is present.
