There are several possible reasons. Steroids and drugs can definitely cause heart damage, by either dilating your heart walls and/or causing the blood vessels supplying your heart to spasm. In either case, athletes place huge demands on their hearts to begin with, and the added stress along with these abnormalities can set them up for a heart attack or dangerous heart rhythm.

Other things that can (and have) led athletes to die aren't drug-related at all, but conditions that are inherited. A common one is called hypertrophic cardiomyopathy (HOCM), which basically is excess but disorganized muscle in the heart wall. This can lead to not only problems with blood flow and the pumping process, but also because of the excess muscle, more oxygen demand to the heart. Again, these are athletes and they already put their hearts to tough conditions.

Another finding (though much rarer) in young people (athletes included) who die unexpectedly is called a "long QT syndrome." Special tissue in your heart conduct electrical currents in a cyclic pattern that cause the muscles to contract and you have a heart beat. In people with a long QT, the end part of the cycle is slower, and when placed in a situation where you need fast beating (ie, physical activity), there is a higher chance that these individuals' cycles go haywire and they end up with a dangerous heart rhythm, leading to disorganized or no blood flow to their organs.

So what can be done? Well, that's what pre-sport physicals are all about. If a doctor can hear a murmur, they can figure out whether or not it may be dangerous, and if there's a suspicion that it is, it is necessary to do further testing. If a condition like this is found, the usual recommendation is not to engage in competitive sports.

The problem is that, I bet most of these competitive athletes already knew they had a heart problem, but in light of the opportunity to achieve fame, make vast sums of money, and/or to escape poverty, they took their chances.

Aerobic exercise makes the heart a more efficient stronger pump. Steroids, diuretics and other drug abuse can cause enlarged heart and heart attacks.

no, too much exercise would cause injury on the field.
they wouldn't have time to get their water, finish listening to their coach talk, and walk to the locker room.
i could be wrong.
it also might be stuff that they are taking.
steroids, or muscle builders that their body is not used to
