If the incident or reason for your complaint was at a hospital, contact Hospital Administration.
If you receive no satisfaction there, file a complaint with your state medical board.
Prepare your complaint on paper. Cite dates and describe the situation as completely as possible, including times and locations. You may have to send in a formal complaint, but then again, you may be able to speak to someone, and follow that up with a written complaint, listing everyone you have spoken to about it.
Everyone should be polite and helpful, but if you run into a problem, don't let anyone intimidate you. Stand your ground until the situation is resolved to your satisfaction.
You can look for the state medical board online, and might even be able to locate your hospital and look up those who comprise the administration of the hospital you went to.

Your first line should be the Department of Health for your state. Most often they have a form you fill out and that is where you supply any supporting evidence to support your claim. From that point it is usually forwarded to the appropriate state board. The board has the authority to revoke or suspend licenses, levy fines, etc. If it is a criminal offense it would be forwarded to the state attorneys office.

Perhaps the American Medical Association, your insurance company as an adjunct to your compaint, and/or a national licensing organization for doctors/physicians. Good luck to you, and hoping that everything works out.
