I think it's "Hit and miss" which is what it says, for example you throw stones at a tin can, some hit, some miss, it basically means the same as 50/50 if you do something, you stand just as much chance of failing as you do of succeeding.

Depends on the context. Generally it means that if your in a sticky situation it could be a chance that it turns out good or bad. Think of Battleship if you hit you win. If you miss you keep trying till you win.

It could mean many things. Did you "hit" it right off or "miss" it.(when talking with the opposite sex).........or could mean somethine sexual (did you hit or miss? Strike out?)...
