that depends on the tile.. smaller tiles (like 4X4) generally have tabs on them for spacing.. or are already laid out on a fiber backing (like 1x1s in bathrooms) larger ones 12X12 and up you generally use prefab spacers.. some old guys just use a big nail
You never wanna go to wide on a grout line though.. wider you go the more likely it is to crack..

Its personal choice basically.
Some people prefer a large grout line while others want nothing more than a thin outline. When you buy the tile the salesperson can tell you which is the most typical application, but feel free to follow your tastes. Remember, grout is not as durable as tile, so if you opt for extremely large lines, be prepared to have a higher maintenance factor.

6"x6" tile use the 1/8 " seperation
12" x 12" tile use 1/4 " seperation
16" x 16" tile use 1/4 seperation
18 " x 18" tile use 3/8 seperation.
In the hardware stores they sell little whie plastic crosses of different thickneses, and depending on the tile size use what is called out for.
