They may have pre-made forms for this at your local OFFICE DEPOT. Check that out before anything.

If you do something home-made, notarize it.

Include that you reserve the right to repo the car should they slack on payments for 3 consecutive months and that they will lose the first 3 payments if the need arise for reposession. Also referencing the warranty of the vehicle would be wise (as is, etc...)

Try Office Depot though, or look online first if you'd feel better about it before going.
