Nobody has, though many have fooled themselves into thinking they have.

I think I did, but I'm not sure. When I was about 15 years old, I had been given an antibiotic to fight bronchitis ( I have it at least 5 times in the cold months of the year every year.). The pharmacy gave me the wrong medication and I was poisoned. My mother didn't believe that an "antibiotic" could make me dizzy and vomit the way I was, she thought I was acting at first. But when she realized that I wouldn't do that and was not acting, she took me to the hospital. As they were pumping my stomach and trying to get it out, I remember a sort of floating feeling and being on the ceiling looking down at what they were doing to me. I sort of just hung around there for a while. Then I felt like a vaccuum was sucking me back and I opened the eyes in my physical body. I remember the doctor saying that had my mother arrived 5 minutes later, I would have been dead.

I have a few times. i researched it, and after some practice, I was able to do it. I saw everything I can physically see now. Every once in a while I notice an entity that doesn't exist on our plane, but so far none of them have been unpleasant.
