If you were really a doctor you would be asking your medical board for you city. I could be a 13 year old kid skipping school and you want my opinion? OK
Just set up a stand like a lemonade stand and run it from there. Charge smokers $1. to talk to you and sell them munchies to satisy their hand-to-mouth cravings and Valium to take the edge off.

If you're a doctor, I'm the president of the United States.
A doctor would know how to establish a clinic as well as know the requirements involved or know how to obtain the requirements without having to go to a forum full of strangers who are mostly teenagers.
