He was diagnosed with Mylodesplastic syndrome and was getting transfusions often, he could not clott so I'm confused.

Myelodysplastic syndrome as you probably know is a severe bone-marrow disease, that leads to the underproduction of many kinds of blood cells. In addition the cells that are produce are often abnormal. Clotting problems in this are caused by abnormal amounts of platelets, and abnormal platelets.
Generally, pulmonary emboli (blood clots) in the lungs are caused by clots forming in veins in the pelvis and thighs, that break-off (embolize) to the lungs. Cancer patients often have abnormal clotting factors that put them at higher risk for the development of these kind of problems. Your husbands problems is particularly difficult in that he was simultaineously at risk for bleeding (low platelets) and abnormal clotting (malfunctioning platelets and probably other abnormal pro-clotting factors).
