Should I take it at face value? Or laugh it off? Or is there something more? I know he's been doing risky behavior (like not using a condom, and he picks up women... and he's had gonorrhea 4 times), should I be scared for him?

I would take it seriously. Sure, maybe he's joking, but then the worst that could happen is he'll make fun of you for falling for it. But if he really is HIV positive, then telling you about it jokingly is his way of testing the waters to see what your thoughts are about HIV. Let him know you're his friend and you'll be there for him no matter what.

He needs to have his head examined in this day and age. But I am guessing he is bragging more than doing.
