Patients require open heart surgery if they have acute valvular insufficiency, or if the coronary arteries are so occluded that angioplasty cannot restore the patency of the vessel. Angioplasty is not open-heart surgery, and is rather a procedure where a long wire is run through the skin and a major artery into the heart, and then into the blocked coronary artery -- then, a balloon is inflated at the end of the wire, which mechanically expands the blocked artery.
