We had an eating relay...teams of two...run down to a sand bucket ... draw a number that corresponds to a "gross" item that they had to eat...scoot underneath table...then tag the next person to repeat the senario until all items off the table have been consumed.
Gross eating item examples: gummy worms in frosting, slurping blue jello through a straw, bowl of cold spaghetti(worms), green slime (under set jello) with gummi frogs floating in it, hotdog octopuses...heck...even sushi could gross them out while being yummy!
Physical feat...spitting gummy eyeballs the furthest...yuck...best of five. or teams holding an egg between thier foreheads the longest.
Party favors: Tee's printed up with the a Fear Factor logo or facimile...team colors...prizes for first place in points...has to be a winner...maybe the fear factor game set?
You could also have a prize "chest" or table for those reward challenges that they have...Gift certificate for icecream store or movie pass.

Food:
Fry up some sausage (like Jimmy Dean) and once cooked mix it with velveeta cheese. Serve on mini bagel slices or mini bread slices. You can call it "Guts" or "puke" (if puke just add a little corn or peas)
Hearts: make a meatloaf mix and form into small heart shapes. Using a can of cherry pie filling, place 1 cherry and a little sauce in the middle of 1 heart and place another on top sealing the edges. When they bite into the hearts they'll definitely be surprise.
Boogers on a stick: put green food coloring in velveeta cheese and dip pretzel sticks into the cheese.
Bloody fingers: use refrigerated sugar cookie dough and shape into fingers. Place an almond slice (using an egg white wash) to the tip to look like a fingernail and bake. Use a cherry or strawberry glaze for a dip (blood).
Maggot Stew: make any soup or stew and at the very end add bean sprouts.
Pimples: fill cherry or grape tomatoes with whipped cream cheese and have a little oozing out.
Potato bugs: Put some extra thick mashed potatoes in a piping bag with a large tip and pipe out shapes that look like potato bugs.
Dessert:
Eyeballs: you make peanut butter balls with crushed butterfingers in it, then dip in melted white chocolate and use red gel icing to put the "veins" and either green or blue gel for the iris.
Kitty Litter Cake. You can find the recipe on the internet.
Drink:
Just do the typical green sherbert in sprite and pineapple juice. When the sherbert melts it looks gross.
As far as games, I would suggest some of the normal relay games like carrying an egg on a spoon, but use inside of the egg without the shell, the sack race but put either jello or something gross feeling inside the bags, etc....
Hopefully you can use some of the ideas.

I would buy something no kids like to eat like liver or brussle sprouts and that would be the gross things that they have to eat to make it to the next round i would make that the second obstacle
get some type of tub bushle basket something they can bob into fill it with water add some red food dye and put fake bigs snakes spiders frogs and what ever else you can find into it and they cant use there hands only there mouth
get a bike skate board pogo stick jumper ball anything that moves and make them do an obsticle course but they have to get through it using at least 2 of those things they can choose which ones they want to use

For the cake, you could top a regular cake with frosting, then a bunch of crushed chotolate cookies (dirt) and gummie worms, eyeballs, etc...it recalls their gross-out stunt, but is actually yummy!
You could set up an obstacle course in the yard, and time them going through it, you could have them run a race with an egg on a small spoon-the first one across wins the challenge, you could have them ride a bicycle a certain distance while balancing something on their heads-the one who goes the furthest or makes it to the goal in the fastest time wins. They could have to jump on a pogo stick the longest...

try fear factor smoothies which are made from food that will not hurt you its just disgusting combinations like boiled eggs and pepper and poptart or mayo and water and eggplant or cooked chickenlivers and maybe beets. but tell them its some thing else.
or you can have them ... well i don't want to tell you to do any stunts that can get them killed. thats all i can recommend. my brothers and i would climb trees or see who could hold their breath under water or lock our selves in our moms car trunk etc didn't cost a thing. be safe!

-blend up a mcdonalds hamburger in a blender and have them drink it
-buy crickets at a pet store (I ate one once, and I'm not dead yet! So I think they are safe to eat)
