Caffeine is a stimulant and will raise your blood pressure. However, you should see your doctor about your blood pressure. If it is regularly that low that it makes you lethargic, that can indicate a serious problem.

Caffeine raises blood pressure by increasing heart rate; your pacemaker will prevent an increase in heart rate, however. So, it shouldn't raise your blood pressure, unless you drink too much and have an autonomic (sympathetic) response to the caffeine.

The caffeine gets into your blood stream and raises your heart rate. As well coffee may tap into your fat storages better than water. That is why some long distance runners drink a pot coffee in order to tap into their fat storages better for energy...however it makes you pee a lot since it is a diaretic.
