First of all, the Herpes virus lies dormant, typically in the nerves, until activated by- something. Could be stress, could be getting the flu, or a cold, could be lack of sleep it's different for alot of people. the herpes virus then activates,. (Herpes is typically lazy, if you didn't touch someone's cold sore with your shoulder blade, you aren't getting blisters on your shoulder blade...typically...there are, of course, exceptions, especially in small children.) and causes 1st: Reddened, irritated areas on the mouth, genitals, etc.
2nd: small (2-3mm) blisters appear where there reddened areas were
3rd, the blisters can grow and join, making much larger blisters which finally pop and scab over, healing just like you would expect any other scab to heal.

Additionally, the activation of the virus in the nerves can cause great discomfort to the infected person. for instance, some people are able to predict when they are going to have an outbreak because their Sciatic nerve (The nerve that runs down your leg) begins aching or there is a stabbing pain in the buttocks. Still others may know because their skin is extremely sensitive to any stimulus (touch, cold, heat, etc.) for a day or so before the "Shedding" begins.

Herpes is a nasty, evil little bastard of a virus, and I look forward to the day it is vanquished.

I am absolutely terrified and I feel so alone. I just found out last week that I tested positive for hsv1 and 2. I have been with the same man for the past 10 years and within the last year found out that he has cheated on me with numerous women. I am so scared that I can give it to my children by kissing them and them eating with me...I feel like my whole life has changed. And even though the nurses, my doctor, and websites say that a lot of people have it I still feel like I am by myself. My self esteem has dropped and I feel like I have been used up. But now, My life is very happy.

Itchy scratchy. Hurts too.
If you're looking for a way to tell if someone has herpes by looking at them, you can't. Only way is if they're constantly scratching their crotch alot. Even then it could be something else.
