Taco Del Mar is a good one. There was a vegan volunteering with me one day, and she went there even... she said it was one of the only fast food places she could eat.
Taco Bell has some options (7 layer burrito, or bean burrito, or anything else - "hold the meat"). Taco Time is also good... I like the natural veggie soft taco. Burger King has a veggie burger (I tried it though... it's okay but not great) and some of the other burger chains do too. Subway & Quizno's both have good veggie subs. A lot of the coffee shops around here have soup & sandwiches too... A few of the soups are veggie (ya gotta ask though) and they make a mean veggie sandwich! YUM! Jamba Juice is a good one too... all of their stuff (except their Performance Booster) is lacto-ovo-vegetarian, according to the booklet with FAQ's in the store. They don't have much in the way of FOOD... but there are a few bakery things that are really good. I just tried the Honey Berry Bran bar and it was REALLY good.

I would have to say the one with the most variety would be Taco Bell. You can pretty much get anything on the menu with beans instead of meat. Burger King has a veggie burger which I tried when it first came out and it wasn't too good. Perhaps they have made it better. Most "burger joints" offer a gardenburger. Subway has a veggie patty sandwich but I think it has eggs in it. And I read that McDonald's fries are not vegetarian, there is some kind of beef something they are cooked with.

I am a vegetarian and my family go out to eat a LOT. The thing is none of them are vegetarians, I am the only one. So when we do out it is my job to figure out what I am going to eat, because, I myself have had to eat plenty of salads. I'm not saying a good salad isn't something I like every once in a while, but enough is enough.
I have discovered that places like Fazolli's are great. I love pastas. Then there's Wendy's; which has a variety, but very little. Burger King carries a veggie burger with french fries and well some subways have pizza varieties or do what I do and pick off all of the meet, double up on veggies, and try it heated.

My favorite “Fast Food” item is the Seven Layer burrito at Taco Bell.

You can try the veggie bowl at Yoshinoya. I get that with teriyaki sauce, a little soy sauce, a little pickled ginger... yum!
If you're at KFC, the BBQ Baked Beans have only 1 gram of fat, so mayyyybe they're vegan? If so, pair that up with some green beans and corn on the cob (sans butter), and you've got yourself a filling meal.

Very few typical fast food restaurants have any decent vegetarian alternatives. Personally, the two I like are Panera Bread and Chipotle, but they aren't all that big or common. Taco bell has a few choices as well, if you enjoy their food.

Certain McDonalds (the only one I know of in New York, is in the city) are starting to carry Vegetarian Burgers. Like its been said numerous times Burger King also carries one which is really yummy.

Most of the dishes at Noodles & Co (if there's one in your area) start out vegetarian (people can add meat for a little extra cost if they want).

Some places have tofu-dogs, and i know BK has veggie-burger. I usually get a salad or fries in other places that don't have veggie foods.
