Rheumatic fever is most likely to lead to complications when untreated. You have been having treatment so you are less likely to get complications (although it isnt guaranteed). You really need to stop smoking, as that will do serious damage to your heart and lungs which may already have had some damage done to them;

The most severe complication of Rheumatic fever is Rheumaic heart disease (RHD). Death from RHD depends on how severe it affects your heart valves. A two dimentional echocardiogram can help see if your heart valves are affected. If you have RHD then smoking will definitely add to the work of your heart and cause atherosclerosis or clogging of you arteries or even COPD.
