Sometimes to the point of gagging. Especially when they lay flat. What do you think ??

that happens to my grandmother. she gets chronic bronchus due to smoking. it could also be many things including cancer. have her see a doctor pronto. its only going to get worse.

A persistent tickle that engages the gagging reflex might just be an allergic reaction to smoking. Even after 20 years of exsposure, it might be that just now the body is starting to catch up.

Worst case scenario is that this person might be having some early signs of throat cancer, which is probably causing a growth or some kind of foreign presence that is making the tickling sensation. Better check with the doctor and have an examination. I new someone with throat cancer and it was a very ugly, disturbing and disgusting experience for him, he died pretty quickly.

My advice would be to stop smoking and see if that makes any difference. If not, it would still be a good idea to stop smoking, maybe this would be a kind of "wake up" call for your friend.

I think it's time to quit... sounds like bronchial irritations.... I smoked for 25 years and was starting to get that smoker's hack... really hated that and it was a good impetus to quit. Im doing great now (it's been about 13+ years). I think they need to quit Smoking. The cough will eventually go away after they quit .Nicotine is an irritant as well as a stimulant . The smoker will not quit smoking unless he/she wants to. Tell them to try drinking a little honey and lemon before bedtime. This should help with the tickle..

After 40 years of smoking, I ended up with a very violent non-stop cough and asthma and COPD (which is caused by smoking). Time to quit. I used the nicotine patches which gave me the same amount of nicotine as the cigarettes did so I never felt nervous - the patches wean you off.

And now I have to use albuterol/singulair/advair/Spiriva Handihaler every single day for the rest of my life. So, it's time to quit.

Yeah when you smoke you kill the tastebuds in your mouth so why not other cells in the back of your throat. I have several co workers who have smoked for years and every winter they are the first to catch whatever is going around AND they continue to cough all winter after the bug is gone. It's called smokers cough. Tell them to quit for their own good and your sanity.

Sounds like COPD, get to a Dr for some inhalers and maybe 0xygen

Raise their side of the bed or sleep on a double pillow see if that
helps.

Could it be a food allergy and not a usual plant or dust allergy? Try some honey and swallow it slowly - that will take the tickly feeling away.

if they are still smoking have them see their doctor if they have recently stopped have them waight a couple weeks and then go see their doctor if it doesnt get better as it could be from quiting
