ALOT OF SMALL SUSHI PLACES SHARE THEIR COMBINED TIPS SO I JUST TIP AS USUAL.....

The same as any restaurant. Since I was a chef i often tip the kitchen separately to show my appreciation. you could do the same w/ the sushi chef.

15-20% of the bill, depending upon service, before tax, not counting alcoholic beverages. The waitstaff and suchi chef share the tip.

unless you are in Japan, you would tip as normal.
In Japan a tip is considered to be an insult, that they need to hire better staff.
