Visitors who are not such great swimmers will disregard all these rules, plus swim in places with strong currents and no lifeguards.
People wander off the hiking trails and sometimes are literally never found--despite the islands' relatively small size-- they can fall off a cliff in the mountains, down a lava tube, or wander aimlessly on the vast lava fields on the Big Island.
Some visitors think warning signs do not apply to them, and will go over protective fences etc to get that "perfect picture" . One person fell into the "blowhole" and got stuck and drowned--- others were killed by a rock slide at Sacred Falls due to carelessness and disregard for signs--- so many examples.
Then of course, there are water sports accidents, too much partying and no regard for safety, and even the crime-related deaths caused by being too casual and unaware of ones' environment (although actually deaths are rare--- robberies are common)
I doubt Hawaii has any more dangers than any other place where people go to enjoy themselves-- If people get careless, bad stuff happens--- and sometimes it happens no matter what you do to try to avoid mishaps.

If I could choose anywhere to die, I would certainly want to die in Hawai'i. Sure beats being shot to death in Detroit!
