Well, there are different kinds of "meetings".
For many of us, the full moons are when we do work on ourselves and our personal lives; letting go of unwanted emotions or behaviors, doing work to get our lives in tune with the greater pattern of nature, and so on. We renew our connection with each other, honor the iconic personifications of maleness and femaleness (the God and Goddess, the essential polarity of the world), and send energy (raised through dancing, chanting, singing, etc.) towards goals that we set.
And then there are the celebratory meetings, the Sabbats, when we honor the turning of the wheel of the year, the changing of the seasons; when we celebrate harvests and renewal and the goodness of life in it's various circumstances. These are also times that we try to attune our lives to what is going on in the greater world, but on a less personal level - spring is the time for new goals and new plans, summer the time to examine the growth of those goals and plans, autumn the time to look at what we have harvested through our actions, and winter the time to look inward; to reconnect with our inner selves and our community. The Sabbats are more community-oriented, and the Moons more private (though that varies from group to group). Many of us host public (or semi-public) Sabbats, which is what they were in older times - a community gathering to celebrate together.
Moons tend to be private because of the trust required to open ourselves to self-examination.
Indoors or out is a personal choice, though I don't know of many Wiccans who celebrate Beltane or Summer Solstice or Lughnasad indoors. How clement the weather is where you live, and how much grief you're likely to get in your area for being in a public place, will have a big impact on whether you're indoors or out.
Most of us, I think, prefer to be outdoors when we can be. When we cannot be outdoors, we bring the outdoors inside - it is, after all, our connection to the natural world and it's changes that we celebrate.

Depends on the group. But for Sabbats (One of the 8 holidays celebrated by Wiccans thru the year) usually we get together, discuss the coming season, and what we hope to accomplish in the coming days, have some food, and relax.

I'm a solitary so I don't "meet" with anyone actually.
When covens meet, they usually perform some kind of relevant ritual.
