No one loses their mind, really. They sometimes just get a bit overwhelmed in such a fast-paces environment. You are allowed to stop and smell the... homeless people... ocassionally. You can buy a subway map and/or go online and get infomation there before your trip. Subway is the best way (and for me, the only way) to travel in the city. It can get you to the other boroughs and well as to other states, even! I mean, if you have the money to spend then just get a cab and hail for one every time you need to go somewhere but expect to have no money at the end of the day. To make it less stressful, buy an unlimited metro card at any machine in a subway station. They have daily unlimited ones, 2 day ones, weekly and monthly metro-cards. Other than that. prepare for the weather (heavy coat, scarves, gloves in the winter) and don't let the hustle and bustle get you. Enjoy the city that never sleeps and all it's glory. Hope this helps!

I visited NYC this past weekend and had an amazing time. On past trips I had always been intimidated by the subway and avoided it like the plague. This most recent trip though was another story and my gf and I navigated the subways pretty much without any major pitfalls. I used a small pocket guide purchased at Barnes and Noble for like $5. It has both a regular street map as well as a subway line map. Btw, an unlimited daily pass only runs about $7.
As for the taxis, it's best, obviously, to know an intersection nearest your destination. I had a cabie drive past the exact intersection for one of our destinations probably thinking that we were oblivious to our surroundings, it'll save you time and money!

Subway maps are free so you don't have to buy one. Just go down into the subway station and ask the person in the toll collector booth (there are still a few of these left underground) and ask the MTA worker for a subway map.
As Queenie suggested, I would also get a street map because altho the subway maps will tell you how to get to certain neighborhoods and where to transfer to other lines, it is not meant to show street detail.
If you're just exploring Manhattan, you will have no trouble finding your way. (Queens Brooklyn, and Bronx can be a bit more confusing)
The island of Manhattan doesn't run in a true North South direction but for our purposes, N-S works fine:
In the north and south direction you have streets. High street numbers = uptown. Lower street numbers = downtown. After 1st street and below, there are lots of little name streets and this is where you will definitely need a map.
Going east to west in the middle of the island, there are avenues that go from 1-2-3-Lexington-Park-Madison-5-6-Broadway-7-8-9-10.
One thing to remember is that Broadway cuts diagonally from west to east after 59th street, so as you head downtown. this means that the placement of Bway in the avenues i just gave you will change as you head downtown.
Don't worry though! It will make sense when you look at a map!

I was born and raised in Manhattan (30 yrs) so when I moved to Brooklyn, I was a bit confused. I have the Brooklyn version of this map and it is extremely durable and has been extremely useful. 5 bucks is pretty damn cheap too.

Don't panic my friend! Just slow down, look at the subway map and a good street map, and walk around!
Most of NYC is in a decently laid out grid pattern, until you go to the village and to the outter boroughs. Do not worry. It's just like any other place in the world...you need to get comfortable. It takes time. give yourself a break and enjoy the adventure.

Easy, just buy a subway map. They run so frequently...but, you have to bear the smell and people.
The city itself is very easy to figure out once you look at the map. It's all basically just Streets and Avenues that cross each other.
The problem with NYC is that there are too many things to do!!
