First answer. No it isn't weird at all for your MOH to wear the same dress. Some brides choose to have all their girls in the same dress, some choose to have just the maid of honor in a slightly different dress, and other choose for all their bridesmaids to wear the same color with varying styles. It's really a personal choice. If you want your MOH to stand out a little more, have her wear a wrap, and the others not, or have her bouquet slightly more elaborate than the others. Most people will know she is the MOH from where she stands at the ceremony.
Second answer: Yes, she is a little old to be a flower girl. You don't need an even number of girls/guys anyway. Does she know she will be walking by herself without a groomsman to escort her? I think the thought of walking alone would terrify most girls, and some women too! Anyway, she can be a junior bridesmaid at 12, but I wouldn't include her in any age inappropriate wedding activities, such as the bachelorette party or if your shower will be a little bit naughty. She should be told this so her feelings won't get hurt. I think it's a great idea that you want her to look a little more "girly" than your adult bridesmaids too. I think minimal make-up and a simple hairstyle should accomplish this, and definitely LOW heeled shoes.. Maybe she could wear a bow in her hair that matches the dress, while the adult 'maids would have more sophisticated styles.

I have never seen a wedding where the MOH wears a different dress. The bouquet and the fact that she is standing next to the bride lets people know she is the honor attendant.
Also, junior bridesmaids always walk alone unless you have a Jr groomsman. It wont look weird, dont worry. 12 is too old for a flower girl, my daughter is a flower girl this summer and she is 5. Sorry. Just make her a Jr Bridesmaid and she should carry a tussie mussie (mini version of the bridesmaids bouquet).
LOL, When I first read this post I thought you said your MIL was wearing the same dress, could you imagine!!!!

It sounds like there is nothing wrong with the 12 year old's dress-- you don't have to make ANY changes to her except title her a Jr BM (her feelings should be more important to you than "even sides").
Flowergirls are ages 4-9. If I were 12 and told I was FG, I'd be upset, too. Just have the 12 y.o. be a Jr BM-- NO ONE has matching-numbered sides anymore for their wedding-- honestly.
Your MOH can wear the same dress as BMs but have a headpiece of some kind, or have a slightly bigger bouquet than the BMs. (I think having MOH in a different dress is not actually that common-- it's more common to just have bigger MOH bouq.)
I am not crazy about the gloves on the MOH idea, b/c gloves are cumbersome, especialy if she wears rings.

For your MOH question: It is very dated (old fashioned) to have to MOH where a different dress and to wear gloves. Just have a slightly different bouquet for her. Trust me, people will know she is the maid of honor.
For the sister question: You are right. She is too old to be a flower girl and she acting like a 5 year old buying the same dress (why isn't her mother stepping in?). To make her different, consider adding a sash to her dress and having her wear a flower wreath around her head. She does not need to be a bridesmaid, but I would give her lots of "jobs". For example, she could be a candle lighter, read a passage during the ceremony, man the guestbook/gift table, hand out favors or rice after the ceremony, or pass out the programs. Give her a job to make her feel important and have her brother help you with this situation -- and with the mother of the groom who should have stepped in by now.

first, no it's not weird that your MOH has the same dress, but i do like the idea of giving her something special to make her stand out...i would go with a special bouquet. leave the gloves up to her, some people hate wearing them.
as for the flower girl question....she's 12, and your the one getting married...it's really nice that you are looking out for her feelings, but don't worry about her so much! she is probably just jealous of all the attention you are getting and wants to be a part of it.
now, to answer your question...to make her look more "grilie" make sure her hair isn't like all your other ladies...i assume their hair will be done up, so have her's done down or half-down. also, shoes are key. again, ill assume that your bridesmaids will be wearing some kind of heels, so find her some really cute flats.
