They don't. Only the guy from http://www.tourettesguy.com does, and he doesn't really have Tourette's.
Most people with Tourette's don't shout swears or actual words. If they do vocalize as a symptom of Tourette's, it's just sounds (like barking). Usually they'll grimace or make jerky movements only when they're not being treated with medicine.
Don't get me wrong... I find "Tourettes Guy" extremely funny.

people with tourrette's disease usually say curse words, Im pretty sure... Bob Saget is a guy who pretends to have it... Its pretty amusing but disrespectful like so many other funny things...
