Sounds like you bought a "Fee Scheduled" plan from some fast talking insurance salesperson. Lots of these companies are sprouting up in America with too good to be true premiums, and they accept everyone. I have been selling Health Insurance for the past 18 years, and in the past 12 months have been contacted to sell so many of these types of plans- each time I have declined. Even though I would make much more money and honestly they are easy to sell, because the consumer gets ripped off. All I can say is Consumer Beware of any plan that is being offered by any company that is not a household name, stick with the big companies like.
Blue Cross/ Blue Shield, United Health, Aetna, Cigna, Kaiser, Health Net.. or you could end up owing lots of money for claims that are not paid.

Depends on the question...
If you have Cancer Insurance then if it is cancer related you get paid hospital stay..
If you have Heart/stroke insurance and have a heart issue that puts you in the hospital it pays you..
If you have accident insurance then it pays if you have to be in the hospital for an accident related injury
oh or if you are in ICU then there is ICU insurance that pays you..
As a Catch all there is a hospital endiminity policy. It pays for I beleive any type of hospital stay
oh and the prices vary according to reasons and stuff.. most that I have seen are above $75 per day.. some reach around $100 to $300 per day.
There are a few companies.. two of them being Conseco (my personal fav) and Continental
