Let us first start with definition. What is meant by adoption? In Webster Dictionary, we find the following: to adopt is to take into one's own family by a legal process and to raise some one else's child as one's own. This naturally means that an adopted child will no more carry the name of his original father. He or she would, as a result, be cut off from any rights or responsibilities towards his biological parents and would, instead be treated as a full child of the adopting ones.
Does Islam approve of this? As a matter of fact, adoption in this sense was wide-spread in Arabia and the rest of the world when Islam was revealed. In fact, Prophet Muhammad himself had Zayd as his adopted son. He was called Zayd son of Muhammad. Some Muslims lose sight of one important characteristic of Islam and hastily pass their judgment that Islam indeed accepts adoption.
Now while Islam does encourage sponsorship of orphans and poor little ones, so much so that Prophet Muhammad said: "I and the orphan's guardian will be together in Paradise." It also calls upon Muslims to give their best treatment to those wretched children. Even a touch on an orphan's head or a pat on his or her shoulder is a great act of righteousness in the sight of God. Islam also gives the strongest warning to those who devour the property of orphans for they only devour fire in their stomachs and will go to hell fire! (Quran, 1-10).

I just want to add something to **smile**'s answer..it was all correct. In Islam we do not adopt children, we take care of them, we can be their guardian but we can't change their names, they will keep their own name and legally will not be our children
