When a person with this disorder has developed a good relationship with a therapist, sometimes the therapist can request, for instance, "I'd like to talk to Mike now," and the change will be made. It's not automatic. I'm not sure, but it seems that some of the other personalities are in the background listening, and will come forward if they choose to. Sometimes one personality will resist the actions of another, but this is not always successful.
Typically the primary personality does not know about the others, but others are aware of the primary and of each other.

D.I.D usually occurs when a situation is SOOOOO painfully the person escapes - sometimes into someone completely different.
but there is no such thing as Multiple Personalities.
There is at least one mode who is aware of the D.I.D - but usually buried very deep in the subconscious.
On an intersting side note - when Mel Blanc the voice of bugs and many other cartoons was in a coma after nearly breaking all his bones - the doctors were able to communicate with Mel using the voice os Daffy Duck - they would ask Bugs to check and Mel and let the Doctors know how he was doign.

D.I.D. and schizophrenia are not one in the same. Whether one persona knows about the others depends on the internal "system" and if there's communication between the selves. Usually there is a caretaker role or overseer, but not always.
A good therapist will make a difference in improving communication; quieting down more vocal, disruptive personas... or instilling safety within the System.

no. everyone who has this disorder doesn't even know about the other person. Schizophrenia has something similar to multiple personality in which you believe you are talking to people who are not there. That's as close as it comes.
