My kids were all born 2 years apart. (25 mos between #1 and #2 and 23 mos between #2 and #3.) The older siblings always seemed to understand it. We read a lot of books and they went to my prenatal appointments. They helped the midwives find the heartbeat and measure my belly. My midwife drew the baby's position on my belly with my third pregnancy.

I am 2 months pregnant, and my 3-1/2y.o. boy has somehow the idea of me 'having a baby growing in my tummy that will come out when it's ready'.
My 2yo girl has absolutely no idea of what that means, maybe because she can't see any difference on my belly yet.
I have shown them pictures, books, and talked to them about having a new brother/sister.

I would say 3 or 4 years old. That is when my best friends kids realized it. Her oldest is 7 and she is pregnant with her 5th and by now he has a complete understanding.
