Most times all orthodontic procedures will be compiled into one bill. You may be required to pay the office visit cost each time you go in though, which is separate from the actual procedure.

Most orthodontist charge a fee that covers everything. Once that is paid there is no more charges. if you are at a regular dentist that is playing orthodontist you will probably get charged every visit

No. There shouldn't be an additional charge on top of your contract fee.

There may be an additional charge for your retainers when your treatment is complete (ours are included in contract fee). If you have an unusual amount of broken brackets, they may charge you extra. They will also charge for broken or lost retainers.

No--all those monthly visits are included the cost of the braces. Also covered are the brackets if you accidentally break one off---unless you break off too many----but if it's just an occasional one, they should replace it for free.
