The area is cleaned and then both the inner and outer tubes are removed (usual cleaning only involves removing the inner tube). There is no need for the area to be numbed as it isnt painful to do.
The open area (stoma) will be covered with gauze and tape and allowed to close naturally;

Pretty much you just take it out. The stoma usually closes by itself. There really isn't any need for anesthetic. Just be sure to deflate the cuff first (if it has a cuff, some are cuffless).

If the trach tube has been in placed for extended periods (weeks to months), there tends to be some scar tissue build-up... but again, the stoma (hole) will close by itself, but will take a bit longer due to the scar tissue.
