In a urine test when the sugar shows up a lot of times that is when a person is first found to be a new diabetic that they weren't aware of. Their blood sugar is too high and it spills over into the urine. We have done urine test on patients before for symptoms of urgency, frequency, etc....and they don't give us the rest of the story......at that time we do a urinalysis to be sure no urinary tract infection and find sugar in it....then a BS is done (blood sugar) and find it way elevated.
A known diabetic can spill over into the urine if they aren't keeping it in control and spills over into it. A HgbA1c test is done (another blood test) and this shows a history of what their sugar levels have been running up over some time...more than just at the time of the test.

Description

This test is used primarily to monitor urine glucose in people with diabetes mellitus. Diabetes mellitus (sugar diabetes) is a condition that causes problems in controlling the amount of sugar (glucose) in the blood. It is a common disorder that occurs when the pancreas either totally stops producing insulin or does not produce enough of the hormone for the body's needs. This lack of insulin results in a low absorption of glucose, both by the body's cells, which need it for energy, and by the liver, which stores it. Another result is an abnormally high level of glucose in the blood, which is then excreted in the urine

as already mentioned Glucose Tolerance Test and Hemoglobin A1c levels are used. (although the glucose tolerance test is now mostly used to diagnose diabetes during pregnancy)
You can also directly measure the blood glucose concentration and this is routinely done. a fasting blood glucose level higher than 126 mg/dl on 2 occasions or random level higher than 200 is diagnostic.

Glucose in the urine indicates that the blood glucose level has exceeded the kidney's ability to reabsorb all of it (glucose among other things is filtered out initially then reabsorbed through the kidney) This usually occurs around levels of 180 mg/dl. So if you have any glucose in the urine, the levels are 180 or higher.

In the past a urine test was the only available means to guess what your blood sugar was .
The main problem with this is that by testing urine, it tells you what your sugar was not what it is now.
There are still dip sticks available at the pharmacy you can dip in urine to check for ketones . The doctors use them for an initial check.
You can also use these pre treated sticks to make sure your diet soda is really diet ! Until you try, you wouldn't imagine the amount of mix ups in restaurants.

You don't anymore... there are far more accurate ways of determining whether a person is diabetic or not. Glucose Tolerance Test (GTT) and glycosated hemoglobin A1C test, to name a couple.
