Ask him to get circumcised. It will hurt, but it may be worth it later. They won't have to take a lot off, just enough to clear the end off.

You obviously have had this problem for your entire adult
life--Why are you waiting until now to want to repair the
problem? I can't understand how you even have a satisfac-
tory sex life. I guess its something you felt would correct
itself. It won't. You must see any MD to begin with and he
may be able to solve the problem OR he will refer you to a
Urologist. All you need is a minor surgical repair. They won't
cut anything off the end of your penis--They will just open it up.
You may need circumcision--this is only a quick snip of the
foreskin. Quit worrying and do something about it????
Dont get your fiance pregnant unless you are mature enough to raise a family!!!

I'd contact a urologist. Keep in mind the vast majority of people answering on this site probably know nothing about medicine.

(Before I receive a bunch of email from people who do know medicine, instead of harassing me, why don't you provide a better answer to this question than I'm capable...)

I'm a nurse. Schedule an appointment together at a fertility clinic, they are able to assist you with this. Good luck and all the best.

He needs to go see a urologist. This problem can be corrected. Not without a little pain though. If he wants children too, it will be worth it.
