Situation: I am a 58 year old diabetic male and currently on 5 units of Humilin R twice daily together with 25 units of Humiln NPH plus 1 tablet twice daily of Ratio-Metformin.

Morning sugars before breakfast range between 5.8 - 7.4 mmol.

Two hours after breakfast, sugars range 6.5 - 8.7 mmol.

Two hours after lunch, they jump to 10.2 - 13.9 mmol.

Before supper, around 7pm, the range from 8.6 - 9.7 mmol.

What should I do to bring my "after-lunch" sugars down?

It may be time to try a different insulin regimen. They are having good success with a combination of Humalog or Novolog before meals and bed and Lantus at bedtime. You may also need to change your lunch menu around depending on your carb intake and the fat intake. I would recommend making an appointment with your doctor and take your blood sugar history with you.

I am writing to tell you what an incredible impact these methods had on my life! I have had type 2 diabetes for 27 years. For me, the worst part of this horrible disease is the severe pain I constantly get in my feet. The pain is so bad that I avoid standing and walking as much as possible. You might have to increase your units of R in the morning, maybe even your NPH if you take that in the morning. Otherwise, talk to your doctor about Humalog- it's a faster acting insulin that you could possibly take at lunch time. You don't have to wait so long after getting the shot to eat. It acts quickly but doesn't last as long as Humulin R.

Keep your diet in true content. I know it's hard to do but keep tiring. I would suggest you Get a hold of your Doctor or Caregiver and have you start on Lantus. This Insulin is a slow acting once a day injection that if your Diet compares with your eating habits and strong will,... will bring sugars to a resting peace which is hard to substain with someone who is active. Good Luck

give yourself a few extra units and see if it makes any difference, if not try eating a little bit less for your lunch, or maybe have some fruit such as a banana, it has a low glycaemic index and is absorbed that little bit slower

be careful with the carbs your eating at lunch . It helps me to think of 1 half of the carbs as sugars. and adjust my eating as such.
