There are numerous reasons this could happen - you need to see a doctor to rule out any serious problems.
It could be a sign of a bleeding ulcer, or hemorrhoids, or numerous intestinal problems, or cancer.
Most likely it is hemorrhoids, and they are pretty common, but don't ignore it or self diagnose it. Get to a professional to get the right answer.

Numerous reasons. Most often it is just a hemorhoid, as in you are constipated and it makes the skin tear where you poop, and thus, you bleed. If the blood is bright red (meaning that the blood is fresh), usually this is the cause.

Another, more serious reason, is that you could have a bleeding ulcer. Usually with this, the blood will be a darker color (which means that the blood has been in the digestive tract for a while). Often times there will be a strong odor to the poop. You may have abdominal pain.

Another serious reason is a chronic disease, such as a form of colitis, such as ulcerative colitis. These diseases usually also cause diarrhea, and abdominal pain for an extended period. Vomiting may also be present. Blood, in this case, would be darker.

You can buy capsules of stool softener in any pharmacy or drug store on the shelf where the laxatives are sold. Try using them according to the directions for a couple of weeks and see if that doesn't solve the problem. If you are very young and your bowel movements are hard or if you are going to the bathroom only every few days, I'd say it's hemorrhoids. If the problem continues, see a doctor and don't put it off.

The most common reason could be that you are not drinking enough water and while you were pooping, it might have been too hard and make your bowel open up too much and make a cut, or it could be a hemorrhoid.

I did and it was because I have Crohn's disease. I lost so much blood that I had to get a transfusion.

It can be that you are not drinking enough water. Your waste is too hard and it scrapes the lining of your anal cavity as it's passing through.
