EKG is based on the electrical activity of heart muscle. Therefore the heart muscle electrical activity must be altered for it to change. Injury due to low blood flow (angina or myocardial infarction) will cause it to change.

EKGs are a great preliminary diagnostic tool for all sorts of cardiac problems: enlarged heart, arrythmias, blockages, heart attacks, etc. If the electrical impulses are doing something strange, which would alert your physician to a problem, it would show up on the EKG. Now you just need someone competent to interpret it, and voila, you have a reletively accurate picture of your heart.
