Not neccessarily, the person could have high cholesterol levels which can be found in people of all sizes.Cholesterol can be fatal if not controlled.
They could however smoke,drink or be generally stressed out all of which can cause a high BP.I think a person with any of these symptoms should see a doctor immediately.

I know many people with high blood pressure as you say above, that do not take drugs. Stress, alcohol, bad life, high cholesterol, smoking, and other things, can also produce HP.

Could be genetics. Drugs can do it too.
