Well , a very very good question.... In STD's case detection priority is given to the following main diseases :
1. HIV
2. Syphilis
3. Hepatitis B
They have a systematic and specialised test for their detection.
ex : Syphilis (VDRL-Yes they remove blood)
HIV (ELISA - yes they remove blood)
Hepatitis B (Serum level of surface antigen HB-s - yes they remove blood)
Other Stds like : Gonorrhea, Chlamydia, Calymatobacter, etc etc can be deected by the following basic test :
1. MICROSCOPY on slide of the smears taken from Genitals
2. Culturing : on agars of the discharge commonly present in these diseases
3. Animal Inoculation (only for research)

Yes, most test that are used to determine STD are done by taking blood and then culturing it to see. Some test are done by swabbing the mouth or vagina with a sterile Q-tip and then culturing it to see.

in some cases they do an oral test, urine, swab.
