First mistake you just proved you are making... "gosh, it's not all that bad". Look, ALL diabetes is "that bad" They call it the "silent killer" because that's exactly what it does. :People who often don't even take the trouble to find out they have type 2 diabetes, never know about it because for the longest time there are no symptoms. Then, by the time the person starts having those symptoms, the diabetes is already well on the way to destroying their body organs. The last stages of uncontrolled, undiagnosed diabetes are.... well, you don't wanna know..
You don't HAVE to "classify" diabetes as "uncontrolled" If your blood sugar is up there off the charts, nobody should have to draw you a picture. You must have had it diagnosed in the first place, which means that there is a doctor out there somewhere who knows your case. See that doctor again - or any doctor for that matter - and get re-evaluated, and then follow that doctor's instructions to the letter.
If you are a younger person, they are traditionally the ones who have Type 1 diabetes, which should have you already under a doctor's care. But these days, because of such bad eating habits, younger and younger people are getting type 2 diabetes, and often they do not get diagnosed until the damage to the body has been done and they get into an emergency situation. YOUR diabetic status needs to be defined by a doctor, who would then give you clear instructions tailored to your personal situation. After that how good you do for the remainder of your life entirely depends on how well you follow that professional, and expert advice.

Do you have Type 1 or Type 2 diabetes? If you have Type 1 diabetes, it is very hard to control, and none of the advice you have gotten thus far applies to a Type 1 diabetic. I can offer suggestions with more information, but I must say that you need to direct all medical questions to your diabetes care team as a Type 1 diabetic. They are the ones who know your personal situation and needs. This is something you cannot fix over the internet or overnight.

I think you need a new doctor.
Find a great endo and sign up for diabetes education classes eve if it means going over some of the things you already know about.
An appointment with a nutritionist will help you plan a meal program just right for you.

Do you have the right type of insulin for you. If you don't get it right you may need a different combination or a faster of slower.
So if it's taking you so long why not ask your doctor.
There are several types of insulin:
* Rapid-acting
* Short-acting
* Intermediate-acting
* Long-acting
* Pre-mixed
My brother changed to Lantus and has since then better and more regulated sugar levels.
I can't judge whether this is right for you, but it might be worthwhile it discussing with you doctor. You don't need that many shots anymore, it works for 24 hours.

To help control your diabetes you need to eat 3 meals every day and a high protein snack mid morning, mid afternoon and at bedtime every day. You need to limit your carbs as these turn directly into glucose causing a spike in your blood sugar. Be sure to get plenty of exercise daily and all of these things will help tremendously.
