Infection is always a risk. Another problem that can happen is that the pacemaker does not sense your own heart beat and it actually "competes" with your own heart beat. Another being that when the pacemaker "fires" it does not have enough amplitude to get your heart to respond to it in order to create a heart beat. There have been so many advances in the last few years in the implantation of pacemakers that these complications are very rare. The most common complications these days are pain for a few days after the insertion and infection.

I had a combo ICD/Pacemaker implanted into my chest two and a half years ago, The one main concern after implant is infection, I was given some antibiotics after surgery just to help elliminate any possibilties of infection.
The only other thing I experienced was some pain in the implant area this is quite normal till the scar tissue builds up and then you will hardly notice it is there.

I have had one very good friend and one relative who had a pacemaker implanted. Neither of them had any medical problems afterwards. Of course they were both relatively young (not elderly) so this may have had something to do with it.
