I think dark spots/unwashed appearance on the neck can be caused by diabetes. More specifically Type 2 diabetes.
People with type 2 diabetes make insulin, but their body can't use it. As a result, the insulin builds up and a dark area of skin may develop on the neck or armpit. It's called acanthosis nigricans, or "A.N."

Insulin resistance can cause this. It is also a common side effect of Polysistic Ovarian Syndrom - PCOS. Insulin Resistance (IR) and PCOS are often simultaneous. Your doctor can do tests to determine if that is your problem. Both diseases can be treated with medication and diet changes. Don't bother trying to use skin lighteners or skin bleachers, they won't work. You might also notice dark patches under your eyes, inside your thigh/groin area and under your arms. There might also be little skin "tags" in the same area. They are harmless but can be removed in a doctor's office if they bother you. IR also caused wieght gain that cannot be easily reversed without medication to help.
Go to a doctor and ask for help. Tell him/her your concerns. Good luck!

I've checked this out and there are a number of diseases it could be eg. Addison's Disease, Hyperpigmentation and Scleroderma. There are a number of other diseases this could be depending on other symptoms present.
I'm not sure if this is just a general question or not but any discolouration of the skin needs urgent medical investigation.

really, any sort of liver disease can cause this occurance. get a check up and have them run a pcr test for actual hepatitis c antibodies as with hcv, the alt and ast levels in a normal liver panel are not always elevated-

Could be yellow gaundice, my mum many years ago told me off for not washing my neck, turned out I was jaundiced.
Could also be a number of other problems best thing to get a blood sample taken at Drs that will reveal and underlying problems
