You should ALWAYS file one. If you are owed money from the federal or state govt wouldn't you want that money returned to you? I believe you would.
I turn, if you owe the federal or state govt money, you ARE NOT relieved of this obligation because of your personal negligence or ignorance. So get on the bandwagon with the rest of us paying slaves. It'll keep you out of the slammer. The answer is YES!

In the US, yes; unless your income is below the minimum they set.
